Eligibility, enrollment, dependent coverage and qualifying-life-event rules.
Who Is Eligible
If you are a full-time employee of BearCom who is regularly scheduled to work at least 30 hours per week, you are eligible to participate in medical, dental, vision, life and disability plans, and other additional benefits.
When Coverage Begins
Your elections are effective on the first day of the month following your date of hire. If you are hired on the first day of the month, your benefits will be effective that day.
Eligible Dependents
- Your legal spouse/domestic partner (or common-law spouse where recognized) who is not offered coverage through their own employer.
- Children up to age 26 (includes birth children, stepchildren, legally adopted children, children placed for adoption, foster children, and children for whom you or your spouse have legal guardianship or legal custody).
- Dependent children 26 or more years old, unmarried, and primarily supported by you and incapable of self-sustaining employment by reason of mental or physical disability which arose while the child was covered as a dependent under this plan (periodic certification may be required).
- Dependent coverage
- The IRS does not allow premiums for domestic partner or domestic partner child(ren) insurance coverage to be paid on a pre-tax basis unless you have a valid marriage certificate.
Qualifying Life Events
When a Qualifying Life Event occurs, you have 30 days to request changes to your coverage.
- A change in the number of dependents (through birth or adoption or if a child is no longer an eligible dependent)
- A change in a spouse’s employment status (resulting in a loss or gain of coverage)
- A change in your legal marital status (marriage, divorce, or legal separation)
- A change in employment status from full time to part time, or part time to full time, resulting in a gain or loss of eligibility
- Eligibility for coverage through the Marketplace
- Changes in address or location that may affect coverage
- Entitlement to Medicare or Medicaid
- A court order to provide health coverage for your eligible dependents (QMSCO)
- You can request a change due to a qualifying event in PayCom.
Enrollment
Prepare for open enrollment in PayCom.
- All employees MUST complete the enrollment process in PayCom, even if declining all voluntary coverage.
- Enrollment deadline: October 1 to October 15
- Social Security Numbers and birth dates for everyone you plan to enroll.
- To be covered, a newborn must be enrolled within 31 days of birth (even if you are already enrolled in family coverage) and a Social Security Number should be provided within 90 days.
